Using this site
Browsing this site is subject to these notices. Using the Credolex application is additionally subject to the Terms of Service. Our handling of personal information is described in the Privacy Policy and the Consumer Health Data Privacy Policy.
What the service does and does not check
Credolex is a record-keeping and reminder tool. It stores the dates you enter, works out a status from them, and tells you before something runs out. It does two automated checks and records what they returned: a monthly name comparison against the federal exclusion list published by the Office of Inspector General, and, for a nursing licence you enrol, what the board of nursing reports about it through Nursys e-Notify, run by the National Council of State Boards of Nursing. Beyond those two, it does not verify credentials with any licensing board, registry, laboratory or agency, and it does not obtain the underlying document for you.
Neither check is a determination. A name match against the exclusion list is a prompt to go and look at the primary source, not a finding about anybody. A Nursys answer is only what the board of nursing had reported when it was given. Section 2.6 of the Terms of Service governs both, and you remain responsible for primary source verification wherever a law, payer or accreditor requires it.

The content on this site, including descriptions of federal and state requirements, regulatory citations, F-tags, and any statement about what a facility must track, is general information published for convenience. It is not legal, regulatory, clinical, or compliance advice, it is not a substitute for consulting counsel or your state's licensing and survey agency, and it does not create an attorney-client or consultant-client relationship. Regulations change and are interpreted differently by different surveyors. You are responsible for determining what applies to your facility. Credolex makes no representation that using the Credolex service will result in compliance with any law, regulation, survey standard, or accreditation requirement.
No guarantee of results
Statements on this site about catching a lapsing credential, preventing a survey finding, or saving time describe what the product is designed to do, not an outcome we guarantee. Reminder emails depend on the accuracy of the dates and addresses you enter and on third-party email delivery. Section 2.4 of the Terms of Service governs.
Pricing
Prices shown on the pricing page are current as of the date on this notice, are in United States dollars, exclude applicable taxes, and may change on 30 days' notice as described in Section 6.6 of the Terms of Service. Every facility starts with a 30 day trial; no payment card is asked for to begin one and nothing is charged during it. An annual plan chosen during the trial is first billed when the trial ends; a monthly plan is billed from the day it is chosen and carries no free days. Plan tiers are limited by the number of active personnel stated for each tier. A paid plan is billed either by payment card or by invoice with payment due in 30 days, as agreed in the order; Sections 6.4 and 6.8 of the Terms of Service govern. Refund requests go to billing@credolex.com and are handled under Sections 6.5 and 6.9.
The public demonstration
The demonstration is reached at app.credolex.com/#/demo, with a code we issued you or with a pass the door issues on request. It is one shared environment: everything entered into it is visible to every other visitor and is deleted nightly. Do not enter real personal, facility, or confidential information into it. We record which code or pass was used, which screens were opened and for how long, the IP address and the browser string, and we may limit or withdraw access to it at any time. Section 5 of the Terms of Service governs it.
Requesting access
Submitting the form with a work email address opens a facility on a 30 day free trial and sends a sign-in link to that address; no card is asked for and nothing is charged. Submitting it with any other address is a request only, and does not create an account, a contract, or any obligation on Credolex to provide the service. Either way we will email the address you provide about it, and you may ask us to stop at any time by replying or writing to privacy@credolex.com. Prices, plans and what happens at the end of a trial are governed by Section 6 of the Terms of Service.
Intellectual property
The Credolex name, logo, website design, application, and requirement library are the property of Credolex LLC and are protected by copyright and trademark law. Regulatory text quoted or cited on this site is in the public domain; our selection, arrangement, and description of it is not. You may not copy, scrape, republish, or use the requirement library or site content to build a competing product.
Third-party names
Names of regulations, agencies, standards, and third-party products mentioned on this site are used for identification only. Their use does not imply endorsement of Credolex by, or affiliation with, any agency or organization.
Accessibility
We aim to make the site and application usable with a keyboard and a screen reader, at the text size your browser is set to, and with a dark screen where you prefer one. If you encounter a barrier, email support@credolex.com and we will work with you to provide the information or function you need.
Cookies and tracking
Credolex uses no advertising cookies, tracking pixels, or session-recording tools, and the application carries no analytics. The marketing site uses Cloudflare's privacy-focused traffic counts, which set no cookie and build no cross-site or persistent identifier. The application stores your sign-in session in your browser's local storage, along with a few things about that browser alone: the dark-screen choice, the facility you last viewed, whether you have seen the welcome walkthrough, your last import result, and a demonstration pass if you have one. Typefaces are loaded from Google Fonts, which receives your IP address as part of that request. The Privacy Policy has the full list.
Security reports
Report a suspected vulnerability to security@credolex.com. We acknowledge reports within two business days. We will not pursue legal action against a researcher who tests in good faith, does not access, modify, or exfiltrate data belonging to others, does not degrade the service, uses only the public demonstration environment or an account they control, and gives us reasonable time to fix an issue before disclosing it.

We may update these notices. The current version is always posted here with its effective date.
